Each timeline entry and case claim is tied to something checkable — dated correspondence, an AZDHS complaint or inspection number, an agency response, or a public filing. If we can't source it, it doesn't go in the record.
An incident described in a family's grievance or complaint is presented as reported — what the complainant documented at the time — not as an established finding. We say who reported it and when.
Where a regulator reaches an outcome, we state it plainly — including outcomes unfavorable to the complaint. For North Mountain, AZDHS's complaint survey (Inspection #INSP-0168859, Feb 20 2026) cited no deficiencies; that is stated as fact, alongside the complainant's dated dispute of the review's timeliness. Open matters (the separate denture investigation, the APS policy question) are marked open, not concluded.
Named individuals are limited to leadership and corporate officers identified in public filings or acting in a senior official capacity. Other staff are referred to by role. The affected resident is not named on the public site.
Characterization is opinion, not record. It appears in a clearly-labeled block tied to the sourced entries it draws on — never blended into the documented timeline. Because the subjects are private organizations, we apply that separation with extra care.
